Registration may be completed by mail, fax or Internet. A confirmation letter will be sent upon receipt of the completed registration form and payment. If cancellation of your registration is necessary, you may send written notice to the address above. Cancellations made by September 1, 2004, will be accepted subject to a cancellation charge of $25. Registrants who do not attend the conference or who cancel after September 1, 2004 are responsible for the entire registration fee. Substitutions, however, may be made at any time.
